Introduction: This study aimed to evaluate the usability of the admission, discharge, and transfer module of a hospital information system using expert-based and user-based approaches.
Methods: This cross-sectional study was conducted in 2025 at two teaching hospitals in Neyshabur, Iran. The usability of the system was evaluated using the cognitive walkthrough method by three experts in the field of health information management. The identified problems were classified into six usability dimensions, and their severity was determined using the Nielsen severity scale. User-based evaluation was conducted using the Computer System Usability Questionnaire (CSUQ). The total number of eligible users was 36, all of whom were enrolled in the study using a census method. Data were analyzed using SPSS version 26, and the results were presented using descriptive and analytical statistics.
Results: The cognitive evaluation identified 28 usability problems, more than half of which were classified as major or critical in severity. The highest number of problems (7 problems) was identified in the efficiency and effectiveness dimension. The most severe and least severe problems were related to the error and user satisfaction dimensions, with mean severity scores of 3.26 and 1.47, respectively. The mean overall system usability score from the users' perspective was 5.06 out of 7.
Conclusion: Expert-based and user-based methods evaluate different and complementary aspects of usability. The simultaneous use of these two approaches can provide a more comprehensive evaluation of the usability of hospital information systems and provide valuable evidence for improving the design and development of these systems.
