fa مقایسه هزینه واقعی اعمال جراحی گلوبال با تعرفه‌های مصوب دولتی در بیمارستان‌های دانشگاه علوم‌پزشکی بیرجند در سال 1400 تخصصي Special پژوهشي Research <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Tahoma;"><strong>هدف</strong> یکی از سؤالات همیشگی بیمارستان‌ها و سازمان‌های بیمه‌گر برآورد تعرفه واقعی اعمال جراحی گلوبال است. هدف این پژوهش، مقایسه هزینه‌های اعمال جراحی با تعرفه‌های گلوبال در بیمارستان‌های دانشگاه علوم‌پزشکی بیرجند است.<br> <strong>روش ها</strong> این مطالعه، یک بررسی گذشته‌نگر بوده که در 11 بیمارستان دانشگاه علوم‌پزشکی بیرجند انجام شد. جامعه آماری پژوهش شامل همه صورت‌حساب‌های اعمال جراحی گلوبال در سال 1400 است که معیار ورود به مطالعه را داشتند (وجود حداقل 10صورت حساب) و روش نمونه‌گیری غیر‌احتمالی آسان بوده است. از 91 مورد عمل جراحی 57 نوع عمل جراحی پر تواتر وارد مطالعه حاضر شدند (معادل 63 درصد از کل عناوین اعمال جراحی گلوبال). داده‌های مورد نیاز در مورد هزینه واقعی اعمال جراحی از نرم‌افزار HIS‌ استخراج و با تعرفه مصوب گلوبال در بخشنامه شورای‌ عالی بیمه در سال 1400 مقایسه شد. داده‌ها با استفاده از آمار توصیفی در نرم‌افزار اکسل 2016 تحلیل شد.<br> <strong>یافته ها</strong> ‌بر اساس نتایج به‌دست‌آمده با در نظر گرفتن متوسط هزینه واقعی در هر یک از اعمال جراحی گلوبال، در 52 درصد از کل اعمال جراحی گلوبال بررسی‌شده، درآمد گلوبال کمتر از متوسط هزینه واقعی و در 48 درصد موارد، احتساب هزینه عمل جراحی به صورت گلوبال به سود بیمارستان بوده است. در 56 مورد از اعمال جراحی، هزینه صورت‌حساب بین 12 تا 139 درصد بیشتر از تعرفه گلوبال مصوب بود که در 23 عمل، میزان اختلاف بیش از 50 درصد بود.<br> <strong>نتیجه گیری</strong> نتایج پژوهش حاضر نشان داد بین هزینه واقعی اعمال جراحی گلوبال با تعرفه مصوب تفاوت قابل توجهی وجود دارد و به دلیل بیشتر بودن هزینه واقعی اعمال جراحی، این تعرفه‌ها موجب زیان بیمارستان‌ها شده است.&nbsp;</span></span></div> <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Tahoma;"><strong>Objective </strong>One of the constant questions of hospitals and insurance organizations is the actual tariff of global surgeries. This study aims to compare the actual costs of surgeries in the hospitals affiliated to Birjand University of Medical Sciences (BUMS) with the approved global tariffs.<br> <strong>Methods </strong>This is a retrospective study that was conducted in 11 hospitals affiliated to BUMS. The study population includes all global surgery bills in 2021. The criterion for entering the study was the presence of sufficient samples (at least 10 bills) from each of the performed global surgeries. Out of 91 collected global surgical procedures, 57 surgical procedures (equivalent to 63% of all global surgical procedures) met the inclusion criteria. The actual costs of surgeries were extracted from the hospital information system and compared with the government-approved tariffs. Data were analyzed using descriptive statistics in Excel 2016 software.<br> <strong>Results </strong>Based on the results, 52% of examined global surgery procedures, the global income was less than the average actual cost, while in 48% of the cases, the hospitals benefited from the costs of surgery. In 56 surgery cases, the billing cost was 12-139% higher than the approved global tariff, and in 23 cases, the difference was more than 50%.<br> <strong>Conclusion </strong>There is a considerable difference between the actual cost of global surgeries in hospitals affiliated to BUMS and the approved tariffs.
