fa هزینه فایده دارو و لوازم مصرفی در اعمال جراحی گلوبال شایع زنان در دو بیمارستان آموزشی-درمانی شهر تبریز عمومى General پژوهشي Research <div style="text-align: justify;"><span style="font-family:tahoma;"><strong>هدف:</strong> یکی از روشهای بازپرداخت هزینه های بیمارستانی از سوی بیمه، نظام پرداخت گلوبال می باشد که صحت تعرفه برآوردی بیمه در زمینه دارو و لوازم مصرفی نسبت به هزینه‌های واقعی برای این موارد، مورد سؤال است. هدف از این مطالعه مقایسه هزینه های دارو و لوازم مصرفی اعمال جراحی گلوبال با تعرفه بیمه می باشد.<br> <strong>روش‌ها:</strong> این مطالعه گذشته نگر و از نوع توصیفی- تحلیلی می باشد که در آن به مقایسه میانگین هزینه دارو و لوازم مصرفی در عمل های جراحی گلوبال دو بیمارستان تخصصی زنان و زایمان شهر تبریز، با تعرفه گلوبال برای این موارد پرداخته است. در این پژوهش اطلاعات مربوط به هشت عمل جراحی رایج زنان، در محدوده زمانی سه‌ماهه اول سال 1396 با بررسی 320 پرونده، مورد بررسی قرار گرفت. از نرم‌افزار <span dir="LTR">SPSS</span> برای تجزیه و تحلیل داده‌ها استفاده شد.<br> <strong>نتایج:</strong> مجموع هزینه دارو و لوازم مصرفی در اعمال جراحی سزارین، میومکتومی، زایمان، هیسترکتومی و سقط به ‌طور معناداری بیشتر و در عمل جراحی حاملگی نابجا به ‌طور معناداری کمتر از تعرفه گلوبال برای این موارد می باشد (05/0><span dir="LTR">P-Value</span>)؛ در اعمال جراحی سیستکتومی و کورتاژ تفاوت آماری معناداری در هزینه های دارو و لوازم مصرفی با تعرفه گلوبال مشاهده نشد (05/0<(<span dir="LTR">P-Value</span>. با مقایسه دو بیمارستان، هزینه دارو و لوازم مصرفی بیمارستان <span dir="LTR">A</span> در اعمال جراحی سزارین، زایمان و میومکتومی به طور معناداری بیشتر از بیمارستان <span dir="LTR">B</span> است (05/0<<span dir="LTR">P-Value</span>).<br> <strong>نتیجه‌گیری: </strong>به نظر می رسد تعرفه دارو و لوازم مصرفی در اعمال جراحی گلوبال، متناسب با هزینه های واقعی مربوطه در بیمارستان ها نمی باشد و نیازمند بازنگری جهت جلوگیری از ضرر و زیان بیمارستان های دولتی می باشد.<span dir="LTR"></span></span><br> &nbsp;</div> <span dir="LTR"></span> <div style="text-align: justify;"><strong>Aim:</strong> One of the recent reimbursement methods implemented by insurance organizations is the global system, where the accuracy of estimated tariff by this organization, especially in the field of drugs and medical consumables toward the real costs of the cases, is questioned. The purpose of this study is comparing the drugs and medical consumables&rsquo; cost in global system over the insurance tariff.<br> <strong>Methods:</strong> In this descriptive-analytical study the average cost of drugs and medical consumables of two university tertiary affiliated with gynecological hospitals of Tabriz university of medical science, were evaluated with global tariff; and reasons of different cost of drugs and medical consumables between two hospitals was reviewed. In this research we study 8 common gynecological surgeries for 320 patients in the first quarter of the year, and the related information is extracted from hospital information system (HIS). We used Spss software for statistical analysis.<br> <strong>Results:</strong> The total cost of drugs and medical consumables in caesarean, Myomectomy, accouchement, Hysterectomy and abortion is significantly more than Global tariff, and in ectopic pregnancy the total cost is significantly lesser than the Global tariff for the cases (P-Value<0.05). In Cystectomy and curettage surgeries compared to Global tariff, there is no statistically significant difference (P-Value>005). By comparing the two hospitals, it can be investigated that, in caesarean, accouchement and Myomectomy surgeries the costs of &quot;A&quot; hospital is statistically significantly more than &quot;B&quot; hospital (P-Value<0.05); and in Hysterectomy, Cystectomy, abortion, ectopic pregnancy and curettage, there is no statistically significant difference between the two hospitals.<br> <strong>Conclusion:</strong> It seems that, drugs and medical consumables tariff in global surgeries is not commensurate with its real costs in governmental hospitals. Our findings show that, these tariffs will need to be reviewed in order to avoid financial loss in governmental hospitals.<span dir="RTL"></span></div> بخش زنان و زایمان, بیمارستان, جراحی گلوبال, بیمه.
